Step 2: Submit a copy of your Professional License

Use the form below to submit a copy of your professional license. Please note, your license must be current. Once your license has been submitted, it will be verified by a member of our staff. Upon approval you will be notified, your account will be upgraded, and you will be able to make purchases on our website when logged in.

*Please note it may take a few days to receive approval. Our normal business hours are Monday through Friday, 7am–4pm PST (excluding holidays). Licenses submitted on Friday may not be reviewed until the following Monday.
